Question: I have a question on my right thumb, I'm right handed so it gets a workout. I have had some discomfort in the main joint for some time, frequently I will get a sharp burning pain that extends down the thumb, the thumb also is warmer than the rest of hand. The main joint is enlarged compared to left thumb,and I can't curl thumb at second joint very far only about 50% compared to left thumb. I have tried favering it but it doesn't seem to improve. Thank You!

Involvement of single joint with swelling, warmth and signs of inflammation favors the infective origin, like septic arthritis to be most likely. Gouty arthritis normally affects lower limb joints and autoimmune athyrppathies usually affects more than one joints and can be associated with morning stiffness and signs of systemic illnesses alongside.

Usually, it is caused by bacteria like staphylococci. but rarely many other bacteria may be found to be causative. You can use anti inflammatory drugs like acetaminophen or ibuprofen alongwith antibiotic cover like augmentin or cephalosporin, orally cn be effective bit preferably Intravenous yields good results. Using steroids alone can increase the infection by suppressing immunity.

Meanwhile work up with Xray of affected joint alongwith Complete blood count, ESR , ANA, RA factor and uric acid as first line are to be preferred. Consult an orthopedician if doesn't settles with these measures.
