Suggest Remedy For Absence Of Weight Loss When Having Balanced Diet And Exercise

Question: I am on Clonil 25 mg and Clonotril 1 mg for OCD since last more than 1.5 years. I am better with these medicines. But I have put up lot of weight. I am 5.8" and having weight of 95 Kgs. I am taking balanced diet and also doing regular exercise. However my weight is not reducing. What do your recommend to reduce weight

Hello,

There can be few reasons behind weight gain in your case-

1. Though you work out but it is not sufficient. You can ask your gym trainer to help you with weight strengthening exericse and focus in abdominal area.
If you walk- its better to do brisk walk or jogg inspite of simple walking...which is an overrated exercise and is of little help.

2. Metabolic reason--If you are really doing proper workout - then weight gain can be due to metabolic causes. Get blood sugar levels, TSH, T3 &T4 Levels , lipid profile & liver profile. We neef to correct them if they are disturbed.

3. Dietary- if you feel that your appetite is more because ofdrugs- then take a glass of water before, during and after meals: this will make you feel full despite eating less.

Eat more of vegetables and salads and avoid carbs and fats.

Balanced weight is an outcome of difference between what we eat and what we spend in working /exercise. When it is disturbed due to any reason- either weight increases or decreases.

P S--. in some cases we give medicines for weight reduction when there is no appatent cause of weight gain or no other treatment works.

Thanks for your reply.

I have done all the tests.

1.     It was done in the month of October as part of Life Insurance requirement. However I got results only in December 1st week. Please find attached the same (Pathological Reports_11Oct14). This shows higher level of Cholesterol.
2.     Tests were repeated again. Cholesterol results are almost same. Please find attached the same (Pathological Reports_14Dec14). This time I have got Thyroid also tested. Results were normal (although I could not collect the reports).
3.     I consulted physician and he started following medicines for Cholesterol from 16th December:
a.     Rosuvastatin 10 mg
b.     Esparin 75 mg
4.     I got my Lipid Profile done again on 18th January'15 and results are quite normal. Please find attached report (Pathological Reports_18Jan15).

Kindly suggest next course of action.

Hello,

If your thyroid is normal and now you are already on cholesterol treatment, then only dietary approach and drug are left.

There are few drugs which are used-
Orlistat and topiramate.
Both need normal liver function tests and kidney tests. These tests should be repeated every six months even if they are normal.

These can lead to gastritis in some people.
Orlistat - sometimes can cause fatty stools/ sticky stools- so need not worry if that happens.
Topiramate may cause rash- in that case it should be stopped.

Also,ensure that your Vit B 12 and Vit D levels are normal.

Try to reduce and stop clonazepam - long term use of this drug should be avoided if one can.

P.S. please join a good gym which has a certified trainer- it is really helpful.

Greetings!

a.     I have been going Gym since last almost 1 year (at least 5 days/week). We have good trainer. However he is also not able to identify why weight is not getting reduced.
b.     I have been recommended to take Vitamin D (by Doctor friend). So I am taking D3 XXXXXXX 600,000 IU since last 3 weeks (1 XXXXXXX per week).
c.     B12 has not been done recently. I think it was done few years back and it was very less. I had taken course of 4 or 5 B12 injections at that time. Do you think I should get it done again? Being Vegetarian, generally there are more changes of B12 level going down. Do you recommended any table for B12?

Just a question. Low level of B12 does not allow weight loss?

d.     Clonazepam: If I don’t take Clonazepam, I get very weird dreams and hence I have continued. Do you suggest any other tablet (with little or no side effects) to have calm sleep?

test B 12 & treat; ↑ clonil by 1/2 tab.

Hello,

1.You should definitely get B 12 repeated and correct it if it is deficient. I recommend 5 injections on alternate days of B12 1500mcg & along with tablet nervic 1500mcg for two week, daily.

2.Vit D - tab or sachets 60K IU, one per week for 8 weeks ; along with calcium tablet which has 1000mg of calcium citrate and atleast 100IU of Vit D. The tablet is continued for 4-6 months.

3 you can take zolpidem for sleep in 10mg dosage. The problem here is- that clonazepam also sometimes has good role in controlling OCD...so ,it is tricky in your case. Should removing clonazepam desatbilize you- it will be a blunder then..
you can increase clonil by 1/2tablet and then try to decrease clonazepam.

Discuss this with your doctor. In this way may be you remain good with just one drug!!

4. I think why not you change your gym?? I dont think that proper exercise is not helpful in reducing weight....never heard of it..espically when your metabolic profile is improving and diet is controlled ( as you mentioned).

5. B12 has great role in bringing calmness and relaxed feeling- reducing anxiety level in people. No documented role in reducing weight but..
