Question: Hi Doctor,

I eat clean and healthy deit and workout more than 3 days a week. I take care of my food intake and sleep adequatly. I cook for myself and donot eat outside food, i do not smoke but i drink that too once in a blue moon. Yet i've been noticing a problem for quite some time. I do not have morning erections, my body fat is increasing, i have man boobs and it is difficult for me to gain muscle mass, and my sex drive is reducing. Could you please advise me as to what should i do for it.....

Thanks for writing to us. You are young man and following healthy lifestyle.

You have problem of not having morning erections. You have increasing fat and man boobs as well as decreasing sex drive.

Primarily we need to rule out hormonal dysfunctions.

I recommend following investigations.

- Thyroid level: T3, T4, TSH

- Serum testosterone: free and total

- Serum prolactin level

Go for these investigations in morning with fasting.

- Since when are you facing these problems?

- What is your height and weight? Is there any significant change in your weight in last few months?

- Are you married? If yes provide me details about your sex life. If not married, let me know history of masturbation and/or sex with girlfriend if any.

There is a problem with the server and I am not able to respond through the app. Thus, sending my response over a mail. However I have uploaded the photos in the app, I hope you will find them.

I can get those test done over the weekend.

-Later in December I was having sex and in the middle of the act I was loosing my erection. Since then I've been keen in observing my morning erection and have been noticing that I don't get them..

-My height is 5'10 and my weight is 86 now..
My weight was between 74 to 76 for a very long time but within a period of 7 months it has raised by 10 kgs. Is it possible to gain weight because of my job? I started my job last year in XXXXXXX and have a desk job for 8 hours a day and 5 days a week. But again I've been working out 5 days a week.

-I am not married but i have have a girlfriend and we have generally sex once in a week but it is not great these days, I feel less aroused.. I masturbate twice a week but I have to convince myself to do that

Thanks for follow up and sorry for delayed response.

I have seen photos. It seems that you have more deposition of fat than normal in your chest. Hormonal investigations required if any cause is related to this. If hormonal check-up comes normal it is just due to overweight/obesity.

Your decrease sexual desire can be due to performance anxiety.

Sometime due to fatigue, lack of mood, situational problem or environmental changes man can fail in sex. It is normal phenomenon. However when this failure makes person apprehensive about erection, sex life is affected. This is called performance anxiety. After such failure when person actively thinks about erection or success of intercourse during sex his mind is likely diverted to such thoughts. For constant erection continuous exciting/romantic/erotic feelings should be there in mind. If mind diverted to concern about erection, erection would not be possible.

Absent morning erection does not mean that you have erectile dysfunction also.

For clarification I need hormone reports.

With weight of 86 your Body Mass Index is 27.5. It should be less than 25. You need to reduce weight up to 75 kg. Within 7 months you have gain 10 Kg. Yes it is of course possible due to sedentary lifestyle including desk job. Only working out would not help. You need to control your diet as well as stress if any.

- Do you feel any stress - occupational, emotional or social?

As stress can lead to many hormonal changes, increase appetite and weight gain.

Decrease sex desire can be due to stress. If you are able to do masturbation normally, there is no possibility of any erectile dysfunction.

Your weight gain seems to be overall responsible for your current problem. So first target should be reduction in weight.

Avoid junk and fatty food, reduce overall calorie intake by 25-30% and go for higher level of exercise or running.

Well i do have some social stress... Marriage pressures and family problems...i do not cosume junk at all nor i drink cold drinks ....
