Thanks for writing to us.

I can understand your concern.

At age of 32 most probably erectile dysfunction(ED) is more psychological.I would like to mention treatment aspect of ED.

5- PDE inhibitor group of medicines like sildenafil or tadalafil are used. One of them should be used 2 hours prior to sex. They causes headache,stuffy nose nausea and gastric discomfort like common side effects. Proper physical check up of cardiac system should be done by a doctor before starting the same.These are prescription medicines and you need to consult doctor/psychiatrist/sexologist for the prescription.
Tadalafil is preferred in patients with blood pressure.

You can use another medicine cum vitamin supplement containing pyridoxal-5-phosphate, mecobalamin and l-methylfolate (trade name tetrafol plus)is very helpful in my patients. It has shown improvement in 50-60% patients of mine with ED.You should take it after meal twice a day. If you feel some improvement in 15 days you should continue for at least 2 months. You may get this medicine over the counter as it is vitamin product.

There are some natural lifestyle measures which help to improve blood flow to penis, increase sperm volume and overall sexual performance.

- The most important factor is healthy heart. Blood is pumped in to body by heart.So regular exercise and avoidance of fatty fast food are important measures to prevent heart disease. Regular walking, swimming or running overall improves blood flow to whole body circulation including penis and keep the mind fresh.

- Good healthy food is also an important aspect. Bananas, Garlic, dry fruits(they contains vitamins), eggs, cow milk, cow butter are good food which may help in erectile dysfunction.

- Avoid smoking and alcohol. Low amount of alcohol can help in increasing blood flow but more amount can cause opposite effects. So better to avoid the same.

I started this problem only few mos back say 6 mos.
Also nowadays i am using caverta.

can u recommend good medication for the sperm volume increase & sperm count.

Hi,

Thanks for followup.

You are using caverta(Sildenafil) right now. There is no specific medicine apart from healthy foods whcih is used to increase sperm volume and count in routine.

Sometime testosterone deficiency or low level can cause low volume of sperms. In that case testosterone supplement is given.
You need to go for serum testosterone level check up.

Per ejaculation 2-5 ml of semen comes out. For sperm count you need to go for semen analysis. If report is abnormal then only treatment is required.
Clomiphane citrate is there which is used in infertility however it causes lots of side effects also.

Thyroid hormone check up and blood sugar check up should also be done if there is issue of infertility.

Revert back to me with reports of testosterone and semen analysis.

First of all you need to undergo physical examination and investigations for low sperm volume and count.

- You already have undergone semen analysis.

- Local ultrasound should be done to check anatomical problem if any.

- Serum testosterone and FSH level should be done for physiological cause

According to cause treatment is done.

-In hormonal therapy low dose estrogen testosterone combination therapy may improve sperm count and motility in some men.

-Clomiphene citrate, an anti-estrogen drug designed as a fertility medicine for women is used off-label in men.

-Vitamin E helps counter oxidative stress which is associated with sperm DNA damage and reduced sperm motility,but cannot be effective alone.

Other short term hormonal therapy for infertility and producing child includes human menopausal gonadotrophin (hMG) and human chorionic gonadotrophin (hCG).

Better to consult doctor/urologist once for examination.
