Brief Answer:
Klebsiella is a multidrug resistant pathogen. Culture sensitivity needed.

Detailed Answer:
Hi Mr XXXXXXX

Welcome to health care magic,

I have gone through your query and I understand your worries regarding your wife's repeated urinary tract infection (UTI).

Elaborating more on the organism causing repeated UTI, Klebsiella is a gram negative bacteria belonging to Enterobacteriacae family. This bacteria has the ability to produce enzymes which destroy the antibiotics used causing resistance to treatment.

Based on the details provided in your query, your wife has been treated with various broad spectrum antibiotics.Cefdinir is a third generation cephalosporin.
Ciprofloxacin is a second generation fluroquinolone. Invanz is ertapenem is a first generation carbapenem.

I am elaborating on the generations of antibiotics used because this information denotes the level of resistance in the organism isolated.

Cephalexin is a first generation cephalosporin.

Coming to whether cephalexin is appropriate to be used after using the previously mentioned antibiotics, I would say no.

If the organism isolated is resistant to a third generation cephalosporin then it is doubtful if it will get killed by a first generation cephalosporin.

I suggest the following to be done to avoid repeated infection.

1. Get a repeat culture and sensitivity done of mid stream urine sample to confirm the organism causing infection and its antibiotic susceptibility.

2. A combination of cephalosporin and sulbactum, second generation carbapenems suggested to be used for antibiotic testing.

3. Get appropriate antibiotics prescribed based on the sensitivity pattern and mention to the treating doctor regarding previous antibiotic history.

4. Drinking appropriate amount of water per day to flush out the bacteria causing infection.

5. Cranberry juice is also known to prevent recurrent UTI.

6. Last but not the least, maintenance of genital hygiene.

I hope I have answered your query satisfactorily.

If any more queries, do not hesitate to get back.

Wishing your wife a faster recovery.

Thanks.
