Is TSH Value Of 0.01 A Cause For Concern?

Question: Hi during pregnancy i was diagnoised with hypothyroidism and positive to antibodies...so after delivery my docter suggested to use 25mcg and do a checkup after one month. i could not take a tsh test for 5 months and today tsh value when tested is 0.01 which goes to hyperthyroidsm. Is this something very serious? Please suggest

Presence of anti-thyroid antibodies is observed in Hashimoto's thyroiditis where there is hypothyroidism and also in Grave's disease where there is hyperthyroidism.

Also, during pregnancy subclinical phase can become overt due to the rise in requirement.

Please let me know what antibodies were looked for. You can upload any available report.

The decline in the requirement for thyroxine after delivery and lactation with continuation of the maintenance dose could have caused the low TSH level. However, such low level is a cause for concern and needs further evaluation.

Please see an endocrinologist for a proper appraisal of the situation and plan further management. Thyroxine supplements should be halted with such low hormone levels.

Tpo were tested during pregnancy first trimister. The value is 183.

Reecently i am diagnoised with high bp. Please let me know if this is someyhing very serious ..i am totally worried..my baby is just 6 months old now.

My one more concern is is thyorid levels causing high bp? Also will these effect my baby because of breast milk?

You are right indeed. Disturbances in the thyroid function can influence the blood pressure levels. Secondary hypertension, as it is called in this situation can be seen with hyperthyroidism. As the thyroid function is controlled, the blood pressure also becomes normal.

Breast feeding can be continued with thyroid disorder. The fact is the milk supply is either let down or surplus depending on the hormone status. If you have none of these problems, you need not worry.

The medications used for the treatment can affect the baby's thyroid through breast milk. So, the baby should be periodically monitored for lowered function or a swelling of the thyroid gland. Also, medications are better taken after a breast feed instead of prior to giving the feed, to lessen the effect.
