Is Synacthen Injection Safe For Treatment Of Painful Bout Of Gout?

Question: Iam suffering a painful bout of gout. Allopurinol has had no effect, taken over ten days. I am allergic to aniinflammotories, penicillin and an I identified allergic. My doctor has recommended a synacthen injection. I recently had a fungal throat infection. I am concerned about having this injection, but what is the choice? Help

Brief Answer: Synacthen is unlikely to cause allergic reaction Detailed Answer: Hi Ma'am, Thanks for your query. Allopurinol reduces Uric Acid levels in the blood and is used as a preventive treatment for gout. It has NO role in the treatment of an acute attack of gout. Acute attack of gout is treated with anti-inflammatory drugs and Colchicine. Since you are allergic to anti-inflammatory drugs, you may take Colchicine (.5mg) as follows :- Initial dose = 1mg (that is, two tablets) and thereafter 0.5mg every 4-6 hours. Continue it till the pain and swelling is relieved. However, the possibility of you being hypersensitive to Colchicine, cannot be predicted. So, to test for hypersensitivity, you can take 1/4 tablet initially. If there are no indications of hypersensitivity, you can then take it in doses as prescribed above. Allopurinol SHOULD BE STARTED (and continued long-term, with regular monitoring of Blood Uric Acid levels) only when the acute attack has been cured. Injection Synacthen is a steroid. Steroids have significant anti-inflammatory effects and also have a potent anti-allergic effect. It can therefore be taken safely along with Colchicine. Consult your doctor and apprise him of my opinion. I am certain that he will agree with me and prescribe suitable treatment as advised.
