Posted on Thu, 15 May 2014
Question: Good evening,

My question is whether a possible hyper sensitive adverse reaction such as malaise, chest discomfort etc to the UTI antibiotic "nitrofurantoin" can be identified in blood/urine three months after decease?

Thank you.

Brief Answer:
Not likely.

Detailed Answer:
Hi,

Thank you for your query.

1. It is not likely that nitofurantoin will be detectable in the blood after three months.

2. Patients with coexisting renal and liver disease may have prolonged excretion time of a few days, not months.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Regards.

Thanks for the reply.

I suppose that when trying to detect hyper sensitive reactions such as malaise etc possibly caused by the antibiotic nitrofurantoin it is not the medication itself that is to be identified but the bacteria or antibodies that are produced as a consequence of its use?

If this is the case then how long does such bacteria and antibodies remain in the blood and/or urine?

Brief Answer:
As below:

Detailed Answer:
Hi,

Thank you for writing back.

1. A routine blood test and a blood & urine culture will help rule out ant residual bacteria infection.

2. It is possible to run an assay for antibodies to nitrofurantoin or for metabolites of nitrofurantoin.

3. The antibodies should normally be detectable foe a few months. Negative cultures will rule out any bacteria.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Regards.
