Is MB12 Shot Advisable For Child With Infantile Spasms And Developmental Delay?

Question: My son is diagnosed with Infantile Spasms and he has been going through treatment and therapies for last 1.5 years. As of now, he can't not speak, still has problem keeping his body stable. He was social earlier but now has started avoiding people. He does not give eye contact even though earlier he was very good on that. he does not respond to his name where as he was so quick earlier. Recently a therapist told us that we should give MB12 shots to him so he can improve on development growth. We want to know whether we should give him MB12. Should we get any test done before that? We are just curious because his current neuro paed doctor never told us about MB12. Please advise on following:
1. Can this be taken without prescription?
2. Do we need to get any test done before?
3. Is it really useful?
4. What form it can be taken in apart from shots?
5. Whats the dosage?

Any other thing that we can do to speed up his development apart from current medications and therapies.?

Welcome to the HCM

I understand your concerns. MB12 shots have been definitely found useful in patients with B12 deficiency leading to autistic symptoms which are very similar to developmental delays present in your child. But there is no role if your child has normal B12 levels and has known cause for these delays i.e. Infantile spasms. So here are the answers which may help you:
1. It's never advisable to take any drugs without prescription.
2. I would recommend you to go for blood B12 levels. If it's normal, then there is no need to have them. Also I would recommend you an EEG and MRI Brain to come to a definitive diagnosis of infantile spasms and B12 deficiency. B12 deficiency can lead to atrophic changes in brain which are reversed by MB12 shots.
3. It will be useful only if your child has associated B 12 deficiency and not otherwise.
4. Subcutaneous injections are the preferred method. Other modes are not as useful as injections because it's water soluble and easily excreted and doesn't achieve constant levels.
5. Dose of 64.5 mcg/kg every three days may be administered subcutaneously if required.

Therapies useful in true infantile spasms are ACTH, vigabatrin, prednisone and other antiepileptic drugs.
