Posted on Thu, 3 Dec 2015
Question: Hi doctor,

My last periods was on September 23rd 2015. Myself and my husband made out on Oct 11th(no penetration but ejaculated near vagina).I am a lactating mother of 2.5months old baby.My periods cycle is around 33 to 35 days.i took home pregnancy test on Nov 8th and Nov 9th...they turned out to be negative. Is there any chance that I have ectopic pregnancy. I had little abdominal cramping and leg pain during first week of. November . I have nausea feeling.i don't know if I am imagining things.please advise. Are there any chances that I have ectopic pregnancy.

Brief Answer:
delayed menses, during lactation.

Detailed Answer:
Hello,
Thanks for the query to hcm,
The fact that your your urine pregnancy test on nov 8/9th has been negative it indicates that you are not pregnant and the delay is likely because you are lactating and your prolactin levels are high.
If it would have been pregnancy ( any type- normal or ectopic), urine pregnancy test would have come positive, so rest assured there is no pregnancy at present, but you should follow contraception in form of barrier contraception to avoid any accidental pregnancy simply because you have started your menses post delivery and that you may ovulate irregularly till you are breast feeding and your menses would be irregular till then.
If you are still anxious then you may get a serum beta hcg test done , which if less than 5ng /dl, then pregnancy is ruled out.

Detailed Answer:
Yes serum beta hcg is a blood test.
The abdominal cramping and nausea may be your premenstrual symptoms and likely that you may have your menstrual bleed in next few days as you are already delayed and your pregnancy test is negative.

Detailed Answer:
home pregnancy test is reliable after 8 days of missed periods , but again depends if you have used a reputed company like cipla or preganews.
But on the whole ectopic pregnancy can only be diagnosed , if present through ultrasound transvaginal only , if the Serum beta hcg test or urine pregnancy test is positive and the uterus is empty, then in that case pregnancy is likely to be ectopic.
Serum beta hcg test is more specific and sensitive than urine pregnancy test.
