Hello,
My name is XXXX, I’m 53 and I emailed a question Sunday evening, but since then I looked into what the doctor recommended but since then I’m trying to find an answer to heal the foot.
     I have a sore on the bottom of my foot due to the fifth metatarsal becoming a bony prominence after it had been fractured several years ago and my left is cleft. I had the fifth metatarsal planed down and everything went well for a year and a half then it became a problem again where the graph path had been put on, but since I have a tendency to roll out to the area of the foot where the bone and skin graph had been put on it became a problem again. This happened in middle of May, and I’ve been under treatment for it since. The only problem is the area that has been ulcerated has been lingering on healing. I’ve been off the last two weeks keeping my foot up to let it heal soaking the foot using domeboro and using prisma to help the wound heal quicker and it hasn’t closed up yet. It’s slightly smaller than it was 2 weeks ago, but not closed yet, which is what I’m hoping it would be by now.
I talked to a pedorthist Wednesday August 1, 2017 and I will be getting a shoe modified to work with the foot.
could getting a second opinion be beneficial at this point?
Would getting a skin graph be an option?
I can't take umpteen weeks off waiting for this to close and what do I tell work, I’m trying. I’m very discouraged right now and I don't what to do to get this to heal up.

With such a presentation in my clinic, I would first rule out [Osteomyelitis] by a clinical examination and to confirm the diagnosis you need complete blood count, bone biopsy and a X-ray. If confirmed treatment will be exact antibiotics in vein as per bone biopsy report and surgical repair only and without surgical intervention it will not heal at all so you need to consult with your orthopedic doctor for it.

I had one MRI when this first happened and a second one just before they planned the bone and both times the MRI showed no sign of infection and since the surgery we went 1.5 years of treatment with no problem to the foot. Im under the impression that the insoles combined with the foot rolling out in the shoe is the problem. I have currently seen a pedorthist and consulting podiatrist about to help alleviate that problem.

If I had two MRI's show no infection what are the chances of there being an infection? What are the symptoms?

If there is an infection then why would it close up and stay that way. I mean there wasn't even a pin hole of an opening.

I'm of the impression I have to be careful of the pressure points of the foot and if I keep rolling out to the outside of the foot, what are the chances that the insoles ad shoes aren't the problem?

If I had an infection, why did 2 MRI's show no infection and then close up completely 2 times.

Thanks for your quick follow up question

“As” per your question I may say in such a disorder you may get symptoms like pain in the affected area and swelling and in chronic cases you may not be able to get any symptom and signs at all, so to confirm it you need a bone biopsy first and it is the only test which can confirm your diagnosis.

How would that be different from pressure from the shoe that breaks down and rolling out in that area causing discomfort in that area of the and being infected knowing it had closed up?

“As” per your doubt you can see by changing your shoe if that can solve your problem and secondly we can not say that there will not be any infection in closed up.
