How Can A Blood Clot In The Portal Vein Be Treated?
Question: What is the treatment for a blood clot in portal vein? Should we have our cardiologist handle this
Can this be treated by the non-surgical stent procedure?
We are seeing a very good cardiologist who detected the blood clot which was the cause of fluid retention and shortness of breath - also we wanted to know if he should resume very light physical therapy as his legs are very weak from reconditioning but don't want to dislodge blood clot!

It is usually treated by vascular surgeon or a general surgeon.
There are three main treatment options:
1- Medical therapy which includes anticogulation. It can remoce the blood clot in up to 80% of the cases.
In acute cases (in the first hours), thrombolisis can be done too, by injecting a thrombolytic agent through a trans-hepatic route.
2- Mechanical blood clot removal. This is made through endoscopy.
3- Shunt surgery. This is usually done when endoscopy has failed. A distal splenorenal shunt is usually the preferred surgical shunt. For patients in whom the splenic vein is also thrombosed and surgery is undertaken, splenectomy (removal of the spleen) and other shunt procedures have been performed.
The physical therapy should be attempted after the treatment of the blood clot.
Other possible underlying causes should be investigated too:
- coagulation disorders
- a malignancy (portal vein blood clotting are a common complication of other tumors in other parts of the body), etc..
