Brief Answer:
Spondylottic changes in spine are likely.

Detailed Answer:
Hello,
Thanks for posting your query.
1. The vertigo can be due to positional vertigo or due to spondylottic changes in the cervical spine. This is age related degenerative change and is not likely to be related to the knee problem.
2. The stiffness in the back is due to spondylottic changes in the spine. This can be relieved by hot compress and oral muscle relaxants. Do consult an orthopedician for an MRI scan of the spine.
3. The replacement in knee is not possible till his cough subsides completely. He needs to receive the lung functions first and then plan for knee surgery.
4. The frequent falls are due to vertigo. Knee problem aggravates the disbalance.
I hope this answers your query.
