Question
I'm 24 years old male. I have thyroid and taking 50 mg of thyronorms. I am also a depression and anxiety patient and taking medicine for depression and anxiety from last 19 months. I have been masturbating from past 10 years First i used to do it twice or thrice a week. But with time it increased.
I started masturbating very frequently, every day. After starting years of masturbating to mid period of masturbating i started masturbating everyday. Sometimes 4 to 5 times in a day with 2 days rest. But after that same 4 to 5 time masturbating a day as i had made it my habit.
With time i started facing problems like back pain, fizzy eyes, weak concentration problem, memory problem, weak erection, watery sperm, early ejaculating leakage of sticky thing from penis on simply thing, getting over excited but i continued masturbating as i have made it my habit Watery sperm made me tensed but when i used to stop masturbating for 5-6 days, sperm that used to come out on masturbation used to be thick and good in quantity but after that again watery sperm.
Now from last 8 months i have completely stopped masturbating and i am taking NF CURE with SHAILAJIT EXTRA STRONG twice a day with water from last two month. I took this medicine after browsing net for hours without consulting a doctor as its ayurvedic medicine In Starting it showed very good result, strong erection, thick sperm, good quantity of sperm.

Now I'm facing a serious problem, from last 15-18 days i am getting nightfall even at day times when i sleep in day hours. Night fall can go up to 3 to 4 times a day which i guess is not normal. I never experienced frequent nightfall in past. What is the reason of experiencing frequent nightfalls?

Now nightfall has stopped as i have stopped taking medicine but now i don't feel anything, i mean i don't get erection even i try to get i cant. I don't get erection in morning when i wake up. I don't get erection during sleep at night. If i get erection, its weak or i loose it few seconds. If i try to masturbate i can't because i don't get hard erection. If me and my partner indulge in oral sex I loose erection easily. If i use condom for precaution in performing oral sex I loose erection. If my partner touches my penis, i don't get proper erection, i ejaculated in few second and sperm is watery n low quantity. Now from last one week i have stopped taking this medicine.

Should i continue taking these medicine? Hope i made you understand my problem as i am not good in English
PLEASE ALSO MENTION

What is the reason of this problem? What is the name of this problem? Can i recover with time? Do i have to take medicine to overcome from this problem? Have i become impotent? Does over masturbation makes you kill your infertility?
WHAT IS
1. Nocturnal penile tumescence (NPT) and how to perform this test at home?
2. Penile Doppler Study
3. Hormone Profile

Brief Answer:
Do not worry and you will well soon.

Detailed Answer:
Hello,
Thanks for your query.
I appreciate your efforts for medical consultation in so much distress.

Main cause of your problems is poor sex education. Masturbation is considered as a part of normal human sexual behaviour and it is very common among young males. It does not have any bad effects on health and does not lead to any medical problems.

Some people have many wrong ideas about masturbation - that it causes ill effects on health like nervous weakness, reduced energy, etc or sexual problems like impotence in the future. All this is not true and these are just myths and misconceptions. If people worried about masturbation or feel guilt after this, then it can lead to anxiety and depression, which can affect erection (like in your case).

Body produce sperm at constant rate. Due to that it is normal to have watery semen (less sperm concentration) on frequent masturbation and thick semen on masturbation on few days gap. If a person do not masturbate, then they used to pass semen in urine or have night fall. Although these are normal response of body, but many persons become worried due to poor sex education (from net/hoardings and quacks). There is no harm in masturbation, but do not feel guilty after it and enjoy it fully during act. Just control its frequency to 4-7 times in a week.

In my view there is no need for Nocturnal penile tumescence and Penile Doppler Study (Done in radiology and andrology labs, not done at home).

Your erection problem more likely due to poor sex education, anxiety/depression and thyroid problem. It is better to get check thyroid hormone and consult psychiatrist for anxiety/depression. There is very fair chance that your problem will resolve.

I hope this information has been both informative and helpful for you. In case of any doubt, I will be available for follow ups.

Wish you good health.

Hello Sir, Below is reply of Dr A XXXXXXX (Sexoloist)
Brief Answer:
Erectile Dysfunction Due to Psychogenic Cause

Detailed Answer:
Hi XXXX,

Night fall IS NOT A DISEASE to be treated.Hence NF Cure is not advisable. If you do not involve in any sexual activity (which ends with ejaculation) for a longer period, it is normal that you get night falls.

What I suggest is to get involved in relaxation programmes like meditation.

If your psychiatrist is confident that your anti depressant medicines are not causing your problem, most probably, you would be suffering from

To assess the cause for erectile dysfunction, you need to be examined and evaluated by Sexual Medicine Consultant.

The investigations to be done are

1. Nocturnal penile tumescence (NPT)
2. Penile Doppler Study
3. Hormone Profile
To rule out physical causes of erectile dysfunction

Brief Answer:
Responses are almost same.

Detailed Answer:
Hello again,
Thanks for follow up.

His reply is almost similar to me:
-Erectile Dysfunction Due to Psychogenic Cause (anxiety/depression).
-Hormone profile. (Your erection problem more likely due to poor sex education, anxiety/depression and thyroid problem. Thyroid secret thyroid hormone)

You do not mention about antidepressant use (mention about shilajeet only). As primary cause appear to psychogenic, then it is not necessary to have Nocturnal penile tumescence and Penile Doppler Study in my opinion.

Wish you good health.

Detailed Answer:
Hello again,
Thanks for follow up.

Do i have to consult sexologist at my place? Erectile Dysfunction Due to Psychogenic Cause? What does it mean?
Answer: In allopathy (English Medicine) nobody is given sexologist degree as single specialist cannot manage all sexual problems. Specialist branch depend on cause of problem. Many times treatment is needed from the team of doctors from different specialties with proper co-ordination between treating doctors.

Most common reasons for sexual problems in young population are psychological. Even if primary sexual dysfunction non psychological in origin but psychological factors like secondary depression and stress exacerbate the sexual problems. In your case masturbation guilt, anxiety and depression (psychogenic cause) are main cause of problem.

Do i have to take medicine for ED? or With time I'm going to recover?
Have i become impotent?Am i going to suffer whole life with this ED problem?
Answer: There is need to manage cause of ED in your case as mentioned in primary reply. In my view you will recover. It is difficult to tell about time.

I'm getting depressed because of this ED problem. If you say i will upload my psychiatrist report. The reason behind my ED problem is side effects of depression medicine? Should i stop taking depression and anxiety medicine?
Answer: Some of the depression medicine may lead to ED. Antidepressants like Bupropion, mirtaz and buspin do not cause ED. These are prescription medicine. You can request your psychiatrist to start them in your case.

OTHER PROBLEM CAUSED BY DEPRESSION MEDICINE
SHAKING OF Hands
WEAK CONCENTRATION and Memory
EXPERIENCING JERK WHILE SLEEPING
Answer: These are more likely symptoms of anxiety and depression rather than side effects of medicines.
