Question: I have 78 kg weight my height is 170 cms. My age is 42 yrs. I want to reduce my weight to IBW i.e 70 Kgs. Presently I have arthritis in both knees, my sugar level is 79 and 189. an am taking lipocut and roseday-F tablets. I have kidney stone in both kidney 8mm and 5mm in left and right kidney respectively.

Thanks for the query.

Overweight, Impaired glucose tolerance, dyslipidemia (as you are taking anti-lipidemics), and renal stone of? Unknown origin are your problems.

Before I tell for weight loss. I would like to suggest to evaluate for the renal stones also - what has caused them and if they have a tendency to multiply in number or size... Please get your serum uric acid, serum creatinine, serum calcium, serum phosphorus, serum albumin and Alkaline phosphatase (ALP) as they are very primary tests to be done. Till that sufficient fluids in a try to flush the stones out should be tried.

About sugars - you are at risk to develop diabetes. But the good news is that you can avoid that as long as more than 25 years, if you really take care of the diet and exercise I suggest you here. You do not need medicine to treat these sugars as of now. So just remind yourself every time you feel like giving up that you should not; because you can avoid diabetes and its complications successfully.

For cholesterol (dyslipidemia) continue Tab Roseday-F. I am not sure how much Lipocut (orlistat) is helping you for weight loss.

Coming to weight loss; the general principles are cut down total intake of food by about 30 %. Give away easy calories / hidden calories from sweets, butter, ghee, fried items, fruit juices, artificial fruit drinks, sugarless-labelled items (e.g. sugar free chocolates), cholesterol free labelled items (i.e. potato chips, mixtures), soft drinks, bakery products where lots of hidden and unhealthy fats are used.
Then what to eat? Chapatis, steamed rice (no biryanis/pulav) with vegetables or minimum gravy of veg/nonveg- for your three main meals. The chapati should not be more than 3 and rice should not be more than 2 bowls in one go. This basic meal has to be accompanied with
A] A half bowl of dal or 1 piece of fatless meat, or 1 roasted fish,
B] A glassful of buttermilk, or half bowl of curd
C] Lots of salads made up of hygienically cleaned cucumbers, tomatoes, carrots (do not worry about the sugars in carrots), radish, spinach, whatever vegetables you can eat raw and sprouts (raw or steamed mixed with the salad). No use of salad dressings. Minimum dressing with lemon juice, salt, little bit of olive oil, pepper, chilli-powder is ok.

Other than main meals, you should eat three small meals composed of
A] 1 to 2 fruits with a few nuts [any fresh fruit and any nut of your likings] OR
B] Egg white with roasted bread and papad OR
C] Homemade bhel made out of sprouts, puffed rice, fried gram, tomato, onion etc OR
D] 1 chapati role with healthy vegetable and little bit of cheese fillings OR
E] 1 bowl of lemon/ tamarind/tomato rice cooked with minimum oil

You can make the combinations of your own choices as per your taste. And you should take about 3 cups of plain skimmed milk spread over these small meals either in tea or coffee or milk drink- all without sugar.

About exercise, please do non-weight bearing exercises by sitting or lying flat on a bed to avoid knee pain and the repetitive movements of your limbs and body should last for 30 to 40 minutes per day. These upper body exercises you can do in front of TV at home even though it's XXXXXXX or raining outside!

So just be determined to help yourself. Losing weight of 8 kgs is very easy and I insist on the same lifestyle even after you lose weight till 70 kg for the best metabolic results.
