She has small loss of Cerebral parenchymal loss which is normal in her age group (although you didn't mention her age but its obvious she is above 50).
Small vascular changes are seen but again its normal to age, as there is no
cerebrovascular disease found.
Dementia and insomnias are sometimes due to hemorrhage in the brain and compression on the brain tissue to make it atrophied and nonfunctional. There is nothing in her like this.
You may
consult a Neurologist for any possibility of dementia. Most important is to examine her physically and performing some tests directly on her.
Take her to a Neurologist and he will do the rest of the job.
