Overall, there are moderate degenerative changes of the lumbar spine with findings most prominent at the L4-L5, where severe degenerative facets, small posterior disc bulge, ligamentum flavum buckling, marginal osteophytes result in moderate bilateral neuroforaminal narrowing. Hypertrophic bony degenerative changes of the bilateral facets resulting in narrowing of the bilateral subarticular recesses with probable contact with bilateral traversing L5 nerve roots. 2. Degenerative changes result in moderate left neuroforaminal narrowing at L5-S1. 3. No significant spinal canal stenosis and any level. 4. Low T1 signal is noted within the facets from L1 through L5, sequela of degenerative facet disease, as evident by sclerosis of the facets on the prior CT abdomen and pelvis from 4/25/2016.
