*SEMEN ANALYSIS REPORT --------------------------------------------------------------------------------------------------------------------- Physical Examination : Quantity 01 ml Colour Whitish pH Alkaline ( 8.0 ) Liquifaction Time 30 Minutes Chemical Examination : Fructose Present Microscopic Examination : Sperm Total Count 86 millions/ml (More than 20 million/ml) Motility 80 % Grade I 15 % (No forward progression) Grade II 15 % (Moving slowly with substantial yaw) Grade III 40 % (Sperm moving in straight line but slower than IV) Grade IV 30 % (Sperm moving rapidly in a straight line with little yaw & lateral movement) Abnormal forms : 06 % Pus Cells 1-2/hpf Epithelial cells Absent RBCs Occasional Impression : Within normal limit.
