General & Family Physician 's  Response
Hi,
From report and history it seems that you might be having chronic urinary tract infection causing recurring attack of UTI.
You require,
1, Urine culture to know causative organisms and antibiotic medicine working against them.
2, ultra sound of kidneys to rule out stones in kidney as there are lenty of RBCs.
After reports go for treatment accordingly.
Take plenty of water.
Keep local hygiene clean and healthy.
Ok and take care.
