What Does Focal Occlusive Soft Plaque In The Report Suggest?

IMPRESSION: Right dominant circulation.- Focal occlusive soft plaque in proximal OM2.- Eccenteric soft plaque in proximal LCX causing approx 30-40% diameter stenosis.sir i want to know the above reports meaning. this is my fathers CT coronary angiography report.

1.The report suggest there is a plaque seen in the coronary vessel - LCX vessel.
2. Soft suggest short duration of the plaque, as in chronic it is seen calcific.
3.The lumen is seen occluding 30 - 40 %, further occlusion may cause emergency.
4.Possibilities of dislodging is there so needful action has to be taken.
5.This conditions can be seen due to systemic causes,like hypertensive / diabetes, so needs to keep the underlying medical condition maintained.
