Hello. Thanks for writing to us at healthcaremagic

You seem to have a form of palmoplantar keratoderma, which commonly presents as dry, fissured skin of the palms and soles. The fissures may pain if they are deep, creating difficulty in walking.

If I was the treating doctor I would have asked you to use an intensive moisturizer frequently and every time you wash your hands.

Topically I usually prescribe a potent steroid+salicylic acid ointment twice daily for the thick fissured skin. Salicylic acid is keratolytic and helps to exfoliate the thick dead skin.
Steroids would bring down the underlying inflammation responsible for the skin changes.

In very severe cases, when topical treatment alone proves ineffective, oral retinoids e.g Acitretin may be started. Retinoids normalize the abnormal keratinization response of the skin and thus bring about improvement in the thick fissured skin.
