Hi..
Can understand your concern..

As per your complain toothache can be due to infection in the tooth that can lead to nerve irritation and inflammation leading to severe pain..

Sore throat can be due to inflammation in throat due to infected tooth while acid reflux caused due to stomach upset that is leading to diarrhea can also be a cause..
Perineal lump can be due to infection while can also be due to over wiping caused due to diarrhea..

For toothache consult an Oral Physician and get evaluated and a thorough clinical evaluation and investigations like x ray can help in diagnosis and treated accordingly..
You can be advised root canal treatment or extraction depending upon condition of tooth..

For other symptoms consult General Physician or Gastroenterologist and get evaluated and blood tests and occult stool test for diarrhea and throat swab culture and examination of the perineal lump has to be done for diagnosis and treatment can be done accordingly..

As of now you can take loperamide for loose motions and also take Ibuprofen to relieve pains..
You shuld do warm saline gargles..
Take Pantoprazole empty stomach in morning..
Take more of cereals, yoghurt and banana in diet..

Hope you get well soon..

Thanks and regards..
