General Surgeon 's  Response
Hi,

The possibilities are:
* Corn that has enlarged from within, deep to cause tenderness and pain
* Possible infection in the cornified tissues
* Viral wart
* Enlarged bone underneath, although this is common at great toe base

Consult a General Surgeon to see for the cause clinically, which will give a proper diagnosis and hence a plan. If viral wart then local application of burn causing solution slowly will help to avoid surgery. If infection underneath then a course of an antibiotic, anti-inflammatory like Ibuprofen. If not then surgery is the best possible treatment.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
