What Causes Swollen And Discolored Testicle To A 18 Month Baby?

My 18 month old grandson has a swollen and discolored testicle discovered when his diaper was being changed. It is the left side, and happened within a couple of hours, as everything looked normal during his last diaper change. What could it be? Does he need medical attention?
