Urologist 's  Response
Hello and welcome to HCM.
As an Urologist, let me assure you that, a hydrocele is a painless swelling.
The hydrocele and cysts can be made out very clearly on an ultrasound scan.
Cysts are formed in the area above the testis, whereas some hydroceles have cystic formations within it,(septated hydrocele).
If you're in doubt, you're welcome to send it here, in my name, for an expert opinion.
