My 89 yr old mother suddenly cannot hold her head in a normal position. She presents swelling of anterior neck and extremities. She has dementia making this more difficult. She has symmetrical pupils an BP of 130/86. Recent medication change for low thyroid is 25mg of synth horrid. Any suggestions?
Hi, Welcome to HCM As it appears from the history given, the major complaints are Swelling over anterior of neck and on the extremities. Considering her age and thyroid status, here are some following Likely conditions with the similar presentation Swelling due to thyroid disease, Cardiac condition Renal condition
A history regarding the associated symptoms of breathlessness, swelling over legs, pitting of the edema on pressure,etc if present, are suggestive of cardiac condition. A generalised edema over face and body is more suggestive of renal cause. Considering her thyroid status, I would suggest a repeat of thyroid function tests, 2d Echo to rule out cardiac cause and renal function test fro renal cause. A consultation to local general physician is very much advisable.
