What Causes Sharp Pain Near The Shoulder Blade While Rotating The Thumb?

I have had a shock like pain in my shoulder near shoulder blade in the soft tissue above armpit opening exist when Irotate my thumb inward/downward and shock like feeling sustains as I hold the position. If I relax the muscle in my left arm and curl my handaround something i can grip with fingers and lift my body up on tippy toes isthe most painful deep socket like shock as long as I hold the position. Been to a Physical therapist for 4 weeks and still no change. Doctor removed to pieces of bone causing what was said to be impingment, the original pain before surgery still exists as the muscles have loosend up to 80% full range of motion.

General & Family Physician 's  Response
Hello and thank you for your question.
Your doctor may need to do some further testing to find what is causing this. The sensation you describe sounds like a nerve is being irritated or stretched when you are doing those maneuvers with your thumb. Sometimes nerve conduction studies or MRI are needed to find the source of the nerve impingement.
