What Causes Severe Swelling In Ankles?

I have a swollen ankle and foot where you can hardl see my ankle bone. I have no injuries to it . I stand a lot during the day on a concrete floor and wear very good insoles and new supporting sneKers every day. This just started to happen. I am 59 yrs young, in pretty good shape , have been doing physical therapy for back spasms but that s seems to be correcting itself. It goes down at night but by the end of the work day all returns. What could be causing this?

As for the explanation for the swelling it could be due to various factors, poor blood circulation, bone related conditions like arthritis, varicose veins, heart related abnormalities, blood pressure fluctuations, and so on.
