Most common cause of Moderate to severe Pain in Rt Upper quadarant is Gall bladder stone

as you are saying that you are also having pain on back and on both side than we have to rule out other causes like

Pancreatitis
Acid peptic disease

we have to evaluate for Cardiac cause

Get following test done and consult with reports

USG whole abdomen
Base line ECG
