What Causes Severe Neuralgia Along With Tinnitus And Headaches?

Hello Dr. Rynne, Stabbing pains from nerve ending. He feels like he s being stabbed inside, shocking feeling feels like he s being shocked at very specific spots all over, ringing in ears all day and night. He experiences headaches 24/7 and only eats 1 time during the day due to the feeling of uneasiness what may potentially be side effects of the medicine that he is taking. Worst part besides the stabbing pains that he feels due to his condition is that if he does not constantly take his medications, the shocking sensation always goes from the left side to right side of his body and he passes out cold. He has been to two (2) specialists within the State of Maryland - One at John Hopkins and the other at University of Maryland. With several series of tests, including MRI, the Physicians and other Medical Specialists can t still seem to determine what is wrong with him... He is currently taking the following meds - 3000mg daily of gabapentin 600mg daily of lyrica

I have reviewed your query and here is my advice.

Tinnitus is common issue and most causes are benign and temporary. However it this lasts for longer period then it should be evaluated. Tinnitus can occur due to causes like chronic ear infection, perforated eardrum, Eustachian tube dysfunction, Meniere's disease, TMJ disorders, acoustic neuroma etc. Tinnitus is not a disease in itself but it is a symptom that indicates that there is some underlying illness. I would suggest you to consult an Otolaryngologist and get evaluated and a thorough clinical evaluation and investigations like Audiometry, X-Ray MRI can help in diagnosis and treatment can be done accordingly.
In case if ringing does not resolve then you can be advised medicines like Tricyclic anti-depressant like Amitryptiline, anti anxiety medication like Alprazolam, as well as Multivitamin supplements can be advised.
