What Causes Severe Fatigue While Suffering From High BP?

I am concerned for Dave whose blood pressure is usually 158/89 pulse 75 --- 131/79 pulse 78. He had a stroke March 21, 2016 had ups and down since then and feeling tired. . Lately he has been feeling tired again yesterday his BP was 159/84 pulse 81. Tonight his BP WAS 194/89 PULSE 72. He refuses to go to the ER- He went to bed. I said calmly, OK lay down for a while - and we will measure your BP after you have rested a bit. Thinking that by resting his BP might come down to a safer level. What should I do. I am afraid he could have another stroke.
