it seems like you have a skin infection in the punctured area. Infectious diseases are the leading cause of death in iv drug users, so I strongly recommend you see a doctor or go to a walk-in clinic to get treatment. Skin infections in these cases have a tendency to spread quickly and cause devastating damage. So please get help and do not try to self-medicate. You do not know for sure the cause of the infection and by seeing a doctor you can get the right antibiotic.
It is never too late to get help from drug addiction.
