What Causes Painful Red Rashes Under The Breast And On Hands?

I am 61 years old. I was diagnosed with high gliadin antibodies in 2007. I ve had a rash on my back since Feb 22 ( day after eating bread when I d been gluten free for 11 years.) I went to dermatologist & got biopsy. Came back contact dermatitis . This was after two years of joint pain and a positive rheumatoid factor, ANA and elev send rate. ( Ana and see rate came back normal on repeat). I saw a rheumatologist who put me on prednisone which I was on for 2.5 months. Felt great on 10 mg. after weaning off, I had a profound rebound with the rash and joint pain returning. On May 30, I had a bite of fish with breading. Woke the next day with rash on my neck. I ve been on steroid cream for the rashes but to no avail. Over the past week, I ve developed a serious rash under my breast ( very red) and on the backs of my hands near thumbs. The thumbs are very painful and have developed large nodes on the knuckles. The rash on my neck and back are now worse and yesterday, I developed a similar rash on my face. All these rashes are raised and flakey. The back rash has had weepy yellowish crust. Shortly after developing the rash on my back, I woke one morning and was unable to lift my arms. The left shoulder joint was painful but the pain was Moore pronounced in the muscles of the upper arms. I was diagnosed about 14 years ago with diabetes insipidus from a head injury as a child. I was also diagnosed the Sjogrens because of severe dry eyes. That was ruled out by neg blood tests but the dry and painful eyes persists. My doctor is referring me to specialists in the city but insurance will take a while for authorization. In the meantime, I don t know what to do. I m sooooo itchy and I so much pain. Any suggestions? I need help now.

Hello,

Rashes are in relation with underlying celiac disease with gliadin antibodies and it is immune response mediated.

So far as nodules on the hands are concerned, they are too immune mediated but in this case arthritis also comes into picture.

Further insight with the lab tests of recent antibodies level and rheumatic panel as citrulinated antibodies, antibodies against Fc portion of IgG is mandatory with an expert.

Till that time, ice pack application with anti inflammatory medications will help partial relief till doc visit.
