What Causes Painful Bowel Movements?

I can not remember the last time I had a pain free, strain free, non-spastic clean bowl movement. I just had one not five minutes ago and it felt like death. This has been happening ever sense I got back from over seas service, but has been getting progressively worse over the years. I was drinking a lot then and thought it might be from that. As i look back on my drinking now days it is low to non existent due to the way I take a hang over these days and my bowl movements. Its not worth it. My diet is not the best, but I dont eat fast food anymore. I mean I cant. Its makes everything worse. I thought I might be lactose intolerant and cut out dairy. The symptoms did not change. I thought maybe drink more water and eat more greens when I can. It seems to help but not prevent my painful bowl movements. I dont take drugs, but I do take ibuprofen at the recommended dose, do to the swelling of my lower Intestines when they act up. I do not have blood in my stool unless I strain to hard and cause hemorrhoids, from which I have had. Some times I cant even help the strain because my colon becomes spastic and does what ever it wants to do. I have diarrhea maybe one time a week or more depending on if I have a beer or two. Any time I get surprised or excited I have to have a bowel movement. Any time I have caffeine, I have to have a bowl movement immediately. They all range from not good, to really bad, depending if I am hydrated. My stool never comes out in a few good movements. Its always little hear, pain, pressure, then a little there. That ranges from day to day. I have medical Insurance and intend to find a clinic soon. I don t go to the VA and have not for years now, do to being treated poorly. Dose it sound like I might have IBS? Any ideas? I don t want to live like this any more, but I do not want to be a test monkey and not have the problem resolved ether. Any advice helps. The problems sound to be due to intestine motility related issue, gut flora disturbances or other hormone imbalances at systemic level. Thorough clinical evaluation, series of lab tests, diagnostic colonoscopy are the primary requisites.
