What Causes Pain In The Lower Back?

I had a fall two years ago and been thru physical therapy. I been going to pain specialist for pain medication which help manage pain in my lower back. The pain specialist suggests surgeon. I went to surgeon and said old and chronic pain. The pain specialist medical report statement it s instability spine and disorder sarcasm? Buttock? What does it mean. Do I need to see another surgeon opinion.

I would like to review your MRI report for a more professional opinion. In case of instability surgery may be needed. Regarding chronic pain, I would recommend therapy with Amitriptyline or Gabapentine.
