What Causes One Sided Headaches?

I have been having tension type headaches for 3 years now (which are sometimes worse on the right side of my head). They last all day long, from the instant I wake up to when I go to sleep, it's the intensity that varies. Every time I get tired, the headaches get worse. Constant and/or loud noises seem to worsen it. The top of my head is very sensitive and painful to the touch. I have right shoulder blade muscle pain. It's always tense and hurts when I do physical activities, no matter the intensity. I have bumps (possibly muscle knots?) on the right side of my neck (following my spine). I need help, I've passed a plethora of tests and no one seems to be of help. I also get these painful tingling sensations (pins and needles) across my body (arms, chest, back, neck, head) whenever I get hot (heat, stress, exercise). Could hypthyroidisme be a cause ?
