What Causes Irregular Periods?

Hi there! I am a 45 year old woman. over the past couple of years my periods have been irregular, but my Dr. has thought I am a bit young to be pre-menopausal. We identified that I have hypothyroidism, and she felt taking medicine for that may cause my irregular periods to subside. But it has not changed anything, and for about the past 3 months, I bleed as though I have my period, fairly light, nearly all the time, there may be 3-5 days where I do not bleed, but then I am back to bleeding.
Is this pre-menopause or should I be more concerned?
