What Causes Interscapular Soreness And Intermittent Cramping?

Yes I have a VAD. A right lower abdominal pacemaker and a right subclavian defibrillator. I have had issues with this pump not only rubbing on my rib-cage but under my xyphoid process. Today I woke up with central intrascapular soreness, felt like something was pushing inside to out at my diaphragmatic region to my right of the xyphoid. I also had intermittent cramping or jolts from this area as if my pacemaker was now pacing me above a threshold of me feeling it or this was just spasms from the diaphragm? Help with the differential !!! Please andc
