What Causes Inflammation And Swelling In The Right Nipple?

I am a50 year old man. I have a right nipple that is red, swollen, burns, and very sensitive to the touch. I have seen a dermatologist who prescribed cream for excema. It has been 4 months now and my condition gets better for a day and returns for the remainder of the week. What do you suggest is wrong? Nothing is helping relieve my condition.
