What Causes Increased Heart Beat After Exercise?

When I get on the stair climber my heart rate goes immediately up to 189-193 and stays there for the duration of my exercise. I find it hard to breath and often times cough when I am done. What could that be? I am a 47 year old woman that has had several cases of pnemonia and whooping cough about 6 years ago.
