What Causes High Levels Of Protein And Blood In Urine?

I went for my annual & was told I had extremely high levels of protein in urine. (3/30/2015) Was referred to a Nuphrologist (I go 6/9/2015) I woke up today painfully passing blood in my urine. Not the 1st time this has occurred, but now has me scared. I do urinalysis every month for my dr due to being on narcotics for chronic pain/nerve damage. If there were something wrong , wouldn t my dr have seen it in the past year? It worries me due to family history of heart disease.
