What Causes Dizziness When Suffering From Diabetes And Hypertension?

I am a 50 y/o male. I have history of Diabetes type II, Hypertension, Gout, Cholesterol problem, and Acid Reflux. I am also an EMT - Intermediate and work on an ambulance. I have been dizzy the last 24 hrs. my sugar level was down to 124 last night. My PCP wants it to be in the 120-150 range which is my normal. I just checked my sugar awhile ago it was 154. Trying to rule out what could be causing it. my BP earlier was 114/74. pulse was 89. What could be causing the dizziness?
