What Causes Constant Swelling Of Stomach?

I am 30 years old and as long as I can remember I've been dealing with constipation and my stomach is always swollen. It doesn't matter whether I eat or not or if I do use the toilet. In the morning before I even eat something it is swollen (looks like I'm pregnant) and I feel nausea. What can it be?
