What Causes Chronic Itching And Bruising On The Foot?

Hi hoping you can help I m in northern Massachusetts in blizzard unable to get out I m Neal 63 YO male worked in health care IVD & pharmaceutical industry 45 years. Problem: I have what appears to be a staph top of foot infection & I have underlying monoclonal gammopathy 5X higher than normal IgM (kappa) aka Waldstroms Macoglobulinemia very low IgG gammaglobulin like 220 (approx 1/5 of normal) and almost no IgA. Am now snowed in with severe blizzard here snow plows packed 4ft high snow 3ft wide behind my car I am disabled (spinal injury) so I cannot get out. A last week Wednesday 3-08-17 I suffered some kind of insect bite on top of my foot, I think like a flea bite from a field mouse that got in the house and was hiding behind my stove (its is now killed in a self contained trap and disposed of last week). My foot got very itchy while sitting in front of the stove and I reached down scratched it and the skin bruised very badly its on top of RY foot which is now red festering open pus producing. Have been washing 2x daily with 95% isopropanol and 3% hydrogen peroxide and betadine and applying OTC triple antibiotic ointment (Neomycin Polymyxin B Sulfate & Bacitracin) after couple of days it still looks red festering so I added Mupirocin Ointment 2% USP along with the triple antibiotic ointmnent, I have been loosely covering with regular size band aid. Tonight washed with alcohol soaked in epsom salt warm water then followed above cleaning and ointments. Top of foot is sore during day put on socks and shoes but as much of time as I can I just have on clean socks over the bandage. I did not go to local MD because my health insurance was middle of changing no coverage then but now have coverage and I thought it would clear up. Now there is 4ft heavy wet snow and ice plowed up against driveway and I have no way to clear that and am concerned infection is spreading. I have on hand 4 Azithromycin 500mg tablets recently prescribed by dentist for prophylaxis use prior to any dental surgery procedures (had Rheumatic heart fever as child) and plus immunocompromized (very low IgG). As my health insurance just changed I have to choose and go to new primary care MD and have first appointment 4/2/17. In meantime am thinking Zithromycin may be advisable rather than risk infection spread. It looks like an open staphylococcus infection, red couple of sort of bubbles white pus exudate. Also on hand are 13 capsules Amoxicillin 500mg, 10 Doxycyline 100mg capsules (it was prescribed for sinus infection a month and half ago but did not work and I was changed to Amoxicillin which did work).

As a first line management, you can apply low potency steroid creams like betamethasone for symptomatic relief. If the lesion persists, it is better to consult a dermatologist and get evaluated.
