What Causes Chronic Dizziness, palpitations And Lethargy?

I quite often feel faint or dizzy when walking or standing for any period of time. I also cannot undertake any form of exercise without feeling dizzy and sick. Even walking up a slight incline or a couple of flights of stairs leaves me breathless and often dizzy. I regularly have palpatations, often feel I lack energy. I feel worse if I have to do anything in hot weather although I generally have these symptoms all year round
