Pulmonologist 's  Response
Hello dear, thanks for your question on HCM.
Chest pain on coughing is characteristic feature of pleurisy ( inflammation of pleura ).
Following are the possible causes for pleurisy.
1. Pneumonia
2. Tuberculosis
3. Pleural effusion.etc
So better to consult pulmonologist and get done
1. Clinical examination of respiratory system.
2. Chest x ray.
You are also having sinus problem, this favours possibility of pneumonia more.
As pneumonia is common after upper respiratory tract infection ( sinusitis).
So get done chest x ray to rule out all these possible causes.
