Hi
Cafe au lait spots also can give brownish coffee coloured appearance on skin. But it's very unlikely to be missed by any trained doctors.
Six or more café-au-lait spots or hyperpigmented macules =5 mm in diameter in prepubertal children and 15 mm postpubertal if present than a thorough examination and laboratory evaluation to rule out neurofibromatosis will be useful.

Hopefully it will be of help. I would be happy to help you in any further questions.
Take care
