What Causes Blockage In The Leg After Bypass Surgeries?

I was told that to fix my blockage in my leg they would maybe do a patch below my knee. I have had two bypasses in this leg already. I just came out of the hospital from the doctor opening up the blockages. Now it is blocked again. Could you tell me what a patch would be.
