What causes blackouts while suffering from severe cough?

My husband has chest congestion and an extremely heavy cough. today he has had several times when after coughing hard he gets very quiet for just s few seconds, appears to be contemplating whether he has more to cough up, but then his hands start to shake and he appears to wake up. he says he feels like he blacks out. He has a doctors appointment schedule for early tomorrow morning. should he consider going to the ER or what is causing these short blackouts. they last only a second or two, but he say he feels like they are longer.

Don't worry so much. Usually after or during a strong episode of cough might happen a black out. It is a normal reaction of the organism.

I have RA and have been injecting 7.5 ml once a week followed by leucovorin 5 mg tab 12 hours after methotrexate. I have been suffering from a cough for 3 weeks. My Family Practicianer prescribed medicine for reflux and allergies, since I did not feel sick except the cough. Another week went by and my cough is worse and I am now coughing up some phlem, but not much. So now I am on azithromycin. My blood tests taken a couple of weeks ago all appeared normal except wbc is approaching the level of being low. I have not ran a fever. My question is when should I be concerned that my severe, non stopping cough is due to methotrexate toxity.
