What Causes Bad Breath In Children?

My son (four) wakes up with really bad fish breath. I mean, REALLY bad. He does have a stuffy nose right now, but his breath typically smells like this; it s just worse when he has a cold. Is it from dry mouth? His nose? Is there anything we should be worried about?

Few reasons which could cause this -
1. Bad oral hygiene winch gets better with thrice a day brushing and mouth washes.
2. Foreign body in the nose which might have been missed out in the past
3. Bad lung infection called - suppurative lung disease.
